This guide is for Floridians with Medicare. If you are choosing a Marketplace (Obamacare) plan, see our Marketplace doctor guide instead.

When an office says “we accept Medicare,” it may mean Original Medicare, not your Medicare Advantage plan. Ask about your exact plan by name. We check the plan’s current directory and call the insurer when an entry is unclear.

Original Medicare: doctors who accept assignment

Original Medicare does not have a network. You can generally see any doctor enrolled in Medicare who agrees to treat you. A doctor who accepts assignment takes Medicare’s approved amount as full payment; you pay only your deductible and coinsurance.

A doctor who does not accept assignment can charge you more. In many cases the extra is capped at 15% above the Medicare-approved amount, called the limiting charge. Say Medicare approves $100 for a visit: that doctor could bill you up to $115, and you might have to pay the full amount at the visit. A doctor who has opted out of Medicare is different: Medicare pays nothing, except in emergencies (medicare.gov: does your provider accept Medicare as full payment?). Ask which one applies before your visit.

Medicare Advantage HMO vs PPO networks

An Advantage HMO generally requires network care, subject to exceptions such as emergencies and out-of-area urgent care. A PPO generally allows covered out-of-network services at higher cost. A PPO label does not mean any office must accept you. Verify whether the provider will treat you, what authorization applies and what the plan will pay before arranging nonemergency care.

How to check your doctors before you switch

Use the full plan name, plan year and network. Search the physician's name and office address, then repeat the check for the hospital or facility. A doctor may participate at one office and not another. Save the directory result and date. If the plan and the office disagree, ask the plan which answer is right and get a reference number before you enroll or book treatment.

Example: a Pasco County retiree checks a heart doctor and a hospital

This is a made-up example to show the steps. The people, plan and dollar amounts are not real.

Frank lives in New Port Richey and is looking at a Medicare Advantage HMO for next year. He sees a cardiologist every three months and wants his heart care at the hospital closest to home.

  1. Write down the full plan name and year, such as “Company Y Medicare HMO, 2027.”
  2. Search the directory for the cardiologist. She is listed at her Trinity office, but not at her Tampa office. Frank plans his visits in Trinity.
  3. Search the hospital separately. It is in network. He also checks the group that reads his heart tests, because it bills on its own.
  4. Call the cardiologist’s billing office. “Are you in network for Company Y Medicare HMO for 2027 at your Trinity office? Do I need a referral from my primary doctor?” He writes down the date and the name of the person he spoke with.
  5. Ask the plan about prior authorization for his yearly heart test, so the approval is in place before the appointment.

If Frank had kept going to the Tampa office, the HMO would generally not pay for those routine visits. At a made-up $250 a visit, four visits would be $1,000 out of his pocket.

My doctor left my Medicare Advantage plan, now what?

Read the plan's provider-change notice and ask for an in-network alternative. If you are receiving active treatment, ask whether continuity-of-care rules apply and how to request them. A doctor leaving does not automatically give every member the right to switch plans immediately. Medicare may allow a special period for certain significant network changes; confirm your own eligibility rather than canceling first.

Specialists, referrals and hospitals

Check each specialist and facility separately. A primary care doctor's referral does not by itself establish that the specialist is in network, and a referral is not the same as prior authorization. For surgery, verify the surgeon, facility and related services with the plan. Ask the treating office which authorizations it will request and keep the approval details.

Send us your doctor list

Send Covered by Guzman the names and office locations of the doctors you want to keep, plus any scheduled procedures. Tell us which provider is essential and which you would consider changing. We can then compare available plans around those priorities.

We do not offer every plan available in your area. For information on all of your options, please contact Medicare.gov, call 1-800-MEDICARE, or contact your local State Health Insurance Assistance Program (SHIP).

Frequently Asked Questions

Can I see if my doctors are in a Medicare Advantage plan?

Yes. Search the exact plan's current directory, then verify unclear entries with the plan and the provider's billing office.

Can you see any doctor with Medicare Advantage?

No. HMO and PPO rules differ, and providers may not participate or accept the plan's arrangements.

Does Medicare Advantage require in-network providers?

Usually, for HMOs: they generally require network providers for routine care. PPOs generally cover eligible out-of-network care at a higher cost, and emergencies are an exception for both.

How do I find doctors that accept Medicare?

Use Medicare's Care Compare for Original Medicare providers and the insurer's directory for an Advantage network. Confirm the office location.

What does accepting assignment mean?

It means the provider accepts Medicare's approved amount as full payment for a covered service. Your deductible and coinsurance can still apply.

Can I change plans if my doctor leaves?

Not automatically. You can change during the fall Annual Enrollment Period (October 15 to December 7), or earlier if you qualify for a Special Enrollment Period, which Medicare may allow for certain significant network changes.
