Getting married opens a window to change your health insurance, but it does not put both of you on one plan automatically. You may have one employer plan, two employer plans, Marketplace coverage or a mix, and each has its own deadline.
Before you cancel anything, check the deadline, the start date and both of your doctors and medications. The best choice is the option that fits your new household and what each of you can actually get, which may not be the plan one of you already has.
Marriage as a qualifying life event
Marriage opens a Marketplace Special Enrollment Period. You have 60 days after the wedding to pick a plan. If you pick a plan by the last day of the month, coverage can start on the first day of the next month.
At least one of you must have had qualifying health coverage for at least one day during the 60 days before the wedding. There are exceptions, for example if you lived outside the United States or in a U.S. territory during that time. The application checks your situation (federal rule 45 CFR 155.420).
That is why we ask about your coverage before the wedding, not just the wedding date. See HealthCare.gov: Special Enrollment Periods for the official list of events.
When you can add a spouse
| Type of plan | Deadline after the wedding | When coverage starts |
|---|---|---|
| Marketplace (HealthCare.gov) | 60 days | First day of the month after you pick a plan |
| Employer plan | At least 30 days (the plan sets the exact deadline) | Set by the plan; ask the benefits office |
Don’t assume the Marketplace’s 60 days apply to a job-based plan. Ask the benefits office for its deadline right after the wedding. Coverage is not backdated to the wedding day.
If you miss the deadline, you will usually have to wait for Open Enrollment or another qualifying event. Open Enrollment for 2027 coverage starts November 1, 2026; enroll by December 15 for coverage that starts January 1, and check HealthCare.gov for the final end date. Keep your enrollment confirmations and payment receipts.
Employer plan vs. Marketplace options
Ask each employer what it costs to add a spouse, whether your spouse is eligible, when coverage would start and whether there is a spouse surcharge. Compare the real family price, not the employee-only deduction on your paycheck.
An employer offer can affect whether you get Marketplace savings. The affordability test for family members is not simply the employee-only price, so we need the actual offer before we can estimate your savings.
You don’t have to be on the same plan. One spouse can keep employer coverage while the other enrolls somewhere else, as long as each person is eligible for the plan they choose.
How household income changes the Marketplace application
After you marry, the Marketplace looks at your combined household income for the whole year, not just one person’s pay or only the income after the wedding.
Married couples generally must file taxes jointly to get premium tax credits, with a few exceptions. Report how you expect to file, and ask a tax professional if you are unsure.
If each of you already has a Marketplace application, don’t leave them as they are. Update the household so your savings are not based on old information; after 2025, extra credit you received has to be paid back in full at tax time. Our income guide explains the application side.
Doctors, prescriptions and network differences for both spouses
Make one list with both of your doctors and medications. A plan that includes one spouse’s primary doctor may not include the other’s specialist or hospital.
Check each drug by name, dose and pharmacy. A medicine can be covered by two plans at very different prices. Also compare the individual and family deductibles and out-of-pocket limits so you know how costs add up for two people.
What you paid toward a deductible usually does not move with you to another plan. If surgery or ongoing treatment is scheduled, confirm approvals and doctors under the new plan before you switch.
Documents to have ready
Have your marriage date and certificate, current policies, proof of coverage before the wedding, employer offers, household details, expected income, doctors and medications. The Marketplace or employer will tell you which documents it accepts.
A name change may need updates later, but don’t wait for a new ID card to start your enrollment before the deadline.
We help with Marketplace enrollment with each person’s consent, and we never add or change another adult’s coverage without that person’s permission. You will see and understand everything we submit for you.
Compare before replacing existing coverage
Confirm approval, the start date and the first premium before you cancel an old plan. Two dates must line up: the last day you need the old plan and the first day of the new one.
Cancel through the Marketplace or the employer, not by just stopping payment. A missed payment can end a plan on a date you didn’t choose, and it does not open a new enrollment window.
Send us your wedding date, both of your current coverages and any employer offers, and we will compare the options. See also our Special Enrollment guide, network guide and HealthCare.gov: who to include in your household.
Frequently Asked Questions
Can I add my spouse to health insurance at any time?
No. Marriage gives you a limited window: 60 days on the Marketplace and at least 30 days on an employer plan. After that, you usually have to wait for Open Enrollment or another qualifying event.
How long do we have after getting married?
You have 60 days for a Marketplace plan. Employer plans must give you at least 30 days, and the plan sets the exact deadline, so ask the benefits office right away.
Do we need insurance before the wedding to use the Marketplace event?
Usually yes. At least one of you must have had qualifying coverage for at least one day in the 60 days before the wedding. There are exceptions, such as having lived outside the United States during that time.
Does our coverage start on the wedding date?
No. On the Marketplace, if you pick a plan by the last day of the month, coverage starts on the first day of the next month. Employer plans set their own start dates.
Can we stay on separate plans?
Yes, as long as each of you is eligible for the plan you choose. You still need to update your household and income on any Marketplace application.
Does my spouse's income count if they are not enrolling with me?
Yes. Marketplace household income includes your spouse even if your spouse keeps a job-based plan. Report your spouse and their income as the application asks.
Should I cancel my old plan after submitting the new application?
Not yet. Wait until the new plan is approved, you know its start date and you have paid the first premium. Then set the old plan’s end date through the proper channel.
